Request a Ride

We have 2 certified drivers who provide transportation for Veterans to their medical appointments at the VA Hospitals or other medical facilities. Veterans are required to call the director in our county when they know the date of their next appointment, time, and location. We like to have a 2-week notice if possible. 

When requesting assistance the following is needed:

• A copy of the DD214
• A consent form needs to be signed and notarized for release of information
